Signs Your Wheelchair Seating Needs Reassessment

Wheelchair seating isn't a "set and forget" prescription. Bodies change, equipment wears and needs evolve and when seating no longer matches the person using it, the consequences can range from discomfort to serious clinical risk. Knowing what to look out for helps carers, family members, and support workers know when it's time to request a review.

Physical Warning Signs

Some of the clearest indicators that a seating review is needed include:

  • Sliding or slumping in the chair, particularly forward pelvic migration

  • New or worsening pressure marks or redness, especially over bony areas like the sacrum, hips or heels

  • Visible changes in posture, such as increasing lean to one side or worsening curvature

  • Skin breakdown or slow-healing marks in areas of contact with the seating system

  • Swelling in the lower limbs that wasn't previously present

Functional and Comfort Changes

Seating problems don't always show up as visible marks sometimes the first sign is a change in what someone can or can't do:

  • Increasing difficulty using their hands for tasks like eating or communication devices

  • Reduced tolerance for sitting - needing to get out of the chair sooner than before

  • New complaints of pain or discomfort, particularly if the person has limited ability to communicate this clearly

  • Increased effort or difficulty with transfers

Equipment-Related Signs

Sometimes it's the equipment itself that signals a problem:

  • Cushions that have lost their supportive properties (common after several years of use)

  • Visible wear, tears, or flattening in foam or gel components

  • A chair that no longer fits properly following weight change, growth, or altered muscle tone

Why Timing Matters

Postural and pressure-related problems tend to progress gradually, which means small changes can be easy to miss day to day, particularly for carers seeing the person regularly. This is part of why a scheduled review cycle matters, rather than waiting for a problem to become obvious. For clients with progressive conditions or complex presentations, more frequent review is often warranted.

Left unaddressed, seating problems can contribute to pressure injuries, worsening postural deformity and reduced independence all of which are more difficult and more costly, to manage once established than they would have been to prevent.

What to Do If You Notice These Signs

If you're a family member or support worker and you notice any of the signs above, it's worth requesting a seating review rather than waiting for the next scheduled appointment. A timely reassessment is almost always simpler than addressing complications after they've developed.
